Graduated from the Faculdade de Medicina da Universidade de São Paulo (FMUSP) in 1979, Wladimir Alfer Jr. completed his residency in General Surgery (1980–1982) and Urology (1982–1985) at Hospital das Clínicas da FMUSP, and completed a specialization as a Research Fellow in Urology at the Harvard Program in Urology, under the guidance of Dr. Jerome Richie, in Boston (1987–1988).
Hired by HCFMUSP in 1988, he worked in the Emergency and Tumor Group (Uro-Oncology) and introduced radical prostatectomy at the institution — a technique learned at Brigham and Women's Hospital. He later became a pioneer in total prostate cryoablation and cryoablation treatment for renal tumors.
From 1999, he also began practicing at Hospital Israelita Albert Einstein (Alphaville unit), actively participating in the creation of the Urology Tumor Board and the Integrated Uro-Oncology Clinics. He completed his qualification course in Robotic Surgery in 2008 and has since incorporated this approach into his practice.

Treatment of benign prostatic hyperplasia (BPH) with HoLEP and minimally invasive treatments (Rezum), renal and ureteral lithiasis, urinary tract infections, vasectomy, infertility, and other urinary tract conditions.
